Frailty resilience project

Frailty is often viewed as inevitable, placing significant pressure on hospital systems. This initiative investigates proactive, coordinated care models designed to support independence. In partnership with aged care providers we aim to evaluate early interventions to shift focus from reactive treatment toward prevention and early intervention. By refining these pathways, we aim to reduce emergency admissions and enhance quality of life, seeking evidence-based, scalable solutions for healthier ageing within the Australian community.
