Risk Assessments for New Placements:
This risk assessment template is designed to navigate the risk assessment process for the onboarding of new placement providers for MDHS student placements.
Prior to commencing an agreement with a placement provider, due diligence in assessing any risks should occur. An educated decision on proceeding with the provider can be established based on the outcome of the risk assessment as outlined in this document.
All risks of medium or higher should also be reported to CESAR through the online form
